本研究は、大規模医療情報データベースおよび一般地域住民を対象としたコホート研究において、生活習慣と若年性認知症発症および認知機能低下との関連を検討することを目的とした研究である。
大規模医療情報データベースを用いた生活習慣と若年性認知症発症との関連を検討するにあたり、まず病院由来の大規模診療報酬請求情報を取得し、若年性認知症患者の治療実態について把握するための薬剤疫学研究を実施し、2023年度に公表を行う予定である。また、薬剤疫学研究のプロトコルを作成する中で、国際薬剤疫学会（International Society for Pharmacoepidemiology：ISPE）と国際医薬経済・アウトカム研究学会（The Professional Society for Health Economics and Outcomes Research：ISPOR）の合同タスクフォースから発表された、リアルワールドエビデンス研究の「再現性向上に向けた調和プロトコルテンプレートHARmonized Protocol Template to Enhance Reproducibility (HARPER) 」の和訳を実施し、2023年度に学術誌にて発表予定である。
また、一般地域住民を対象とした生活習慣と認知機能低下との関連の検討については、2022年11月－2023年1月に実地検診を行い、認知機能検査を含む検診を行った。2021年度まではCOVID-19感染禍で検診が中止されていたこともあり、再開後初年度である2022年度の検診参加者は、63名にとどまった。本コホート研究の既存データを用いた解析として、女性の妊娠出産歴と無症候性脳血管障害との関連などの検討を行い、学術総会および学術誌において発表を行った。
This study aimed to examine the association between lifestyle behaviors and early-onset dementia as well as cognitive decline in a large-scale real-world database and a cohort study of the general population.
In examining the association between lifestyle behaviors and early-onset dementia using a large-scale real-world database, we first used the hospital-derived large-scale medical claims database and conducted a pharmacoepidemiological study to reveal the current and historical treatment status of patients with early-onset dementia.
In addition, while developing the protocol for the pharmacoepidemiological study, we translated the Harmonized Protocol Template to Enhance Reproducibility (HARPER) for real-world evidence studies released by a joint task force of the International Society for Pharmacoepidemiology (ISPE) and the International Society for Health Economics and Outcomes Research (ISPOR). It will be published in an academic journal in FY2023.
We conducted a field survey from November 2022 to January 2023 to examine the association between lifestyle behaviors and cognitive decline among the general population, which included cognitive function tests; however, the total number of participants in the surveys was only 63 in FY2022 since they refrained from participating to avoid infection. Analyses using existing data from this cohort study, including the association between female reproductive events and subclinical atherosclerosis of the brain, were conducted and presented at scientific meetings and in academic journals.
