本研究は治療介入を伴わないPositron Emission Tomography (PET) 研究である。精神病発症危険状態（ARMS）の診断基準を満たす20−69歳の成人12名を募集し、新規PETリガンドである[11C]K-2を用いて脳内AMPA受容体密度をPET撮像により定量する。さらに、プロトン核磁気共鳴スペクトロスコピー（proton magnetic resonance spectroscopy; 1H-MRS）を用いて前部帯状回のグルタミン酸濃度を測定、安静時機能的磁気共鳴画像法（rsfMRI）により安静時の各脳領域間の機能的結合性、拡散テンソル画像（diffusion tensor imaging; DTI）により各脳領域間の白質による構造的結合性を測定する。そして、PET検査実施後、18か月追跡する。症状悪化を呈した際に、対面調査を実施し、米国精神医学会による診断基準（DSM-5）におけるいずれかの精神疾患の基準を満たすか評価する。2021年度、予定通り、4名のARMS患者を組み入れ撮像をおこなった。さらに、比較データの取得のため、統合失調症患者10名を撮像し、脳画像データを得た。
In this PET study without any treatment intervention, we are recruiting 12 persons who are aged 20-69 and fulfill diagnostic criteria for at-risk mental state (ARMS). They undergo a PET examination for the measurement of AMPA receptors, 1H-MRS for the measurement of glutamate concentrations, and resting-state functional MRI and diffusion tensor imaging for assessment of functional and structural connectivity, respectively. They then receive follow-up assessments for 18 months. When they show clinical deterioration, they will receive clinical interviews for diagnostic assessment according to the DSM-5. The study protocol was approved by the Certified Institutional Review Board. Four patients with ARMS and ten patients with schizophrenia successfully underwent a PET scan.
