子どもの虐待予防におけるポピュレーションアプローチとしては子どもを妊娠している親や出産後の親など中心に行われている。虐待を予防するためには、近隣住民の声かけやちょっとした支援が重要だと言われている。しかしながら、日本においてはポピュレーションアプローチの方法を記述したものは見当たらない。そこで、一般住民に対してポピュレーションアプローチの方法について文献を整理した上、対象となる近隣住民等からの支援ニーズや要望を明らかにし、ポピュレーションアプローチモデルを開発することを目的とする。
本年度は、まず文献検討や学会参加により活動について情報収集を行った。国内文献については、医中誌およびJDeamにて、「子ども」「虐待」「ポピュレーションアプローチ」または「教育」で検索したが、ほとんどが妊娠中などの人たち向けであった。その中で上田氏による一般市民と専門職を交えた2006年に行われたワークショップの報告があり、ワークショップに先立ち行われたMatayoshi氏による講演では、虐待予防のために働く職種に期待される特徴と支援者に求められる核となる技能について述べられていた。
国外文献では、Pubmedで "child abuse" AND interventions AND "neighborhoods"で検索し、テーマ・要旨等で選定し、3件が該当するとした。その中でRobin J. Kimbrough-Melton氏らの論文では子ども達のための「強いコミュニテイ」づくりを10の原則を学んだボランテイアがアウトリーチワーカーとなり中心的な役割を担い地域の中で段階的活動を行い、地域内の支え合う関係のネットワーク強化を行っていた。
公衆衛生関連の国内学会に参加したが目的とする報告は見当たらなかった。
これらから、虐待予防のポピュレーションアプローチは日本では事例が少なく、海外の事例から計画的な活動が必要と考えられ、その方法についてさらに検討が必要である。
Population approaches for preventing child abuse are mainly focused on parents who are pregnant or after childbirth. In order to prevent child abuse, it is said that it is important to call on the neighbors and give a little help. However, there is no articles describing the method of population approach in Japan. The purpose of this study was to develop a population approach model of child abuse. To achieve this purpose, first, we review literatures of population approach methods to prevent child abuse. Second, we clarify the support needs and requests from neighboring residents.
In this year, we first collected information on activities by reviewing the literature and participating in academic congress. Japanese literature was searched for "Child," "Abuse," "Population Approach," or "Education" in Ichushi and J Deam, and we found that most of the literature described methods for people such as pregnant. Among them, there was a report of a workshop held in 2006 by Mr. Ueda for professionals or general public. In a lecture by Matayoshi prior to the workshop, he stated that the features which were expected for professions working for child abuse, and core skills required of supporters.
Foreign literature was searched for "child abuse", "interventions" and "neighborhoods" in pubmed, and we found three literatures. Among them, the paper which was written by Robin J. Kimbrough-Melton et al showed that volunteers who learned the ten principles of creating a "strong community" for children became outreach workers, played a central role, and performed step-by-step activities in the area. They strengthened the network of supportive relationships in the region.
Based on these facts, there are few cases of population approaches of child abuse in Japan, and systematic activities are considered necessary from overseas cases, and further study is needed on the methods of population approaches to prevent child abuse.
