本前向き臨床研究は倫理委員会で承認され, 現時点で同種造血幹細胞移植を施行した39例が既に登録され, 内3ヶ月以上生存した35例の患者が臨床経過と経時的な抗体価の変化の評価が可能となっている。本研究では内側にBK polyomavirus (BKPyV)のウイルスゲノムを含まないウイルス粒子構造を有する粒子(ウイルス様粒子 : virus-like particle)を用いた抗原をELISAプレートに固相化した測定系を用いて, 患者血清中の抗BKPYV抗体(IgG, IgM)を検出した。また尿中のBKPyVは定性および定量PCRにより検出した。
解析可能な35例中(年齢中央値48歳), 7例の患者がBKPyVによる出血性膀胱炎を発症した(移植後中央値で23日)。7例の内, 移植後に血中の抗BKPyV-IgMの陽転化がみられた症例が3例あり, 入院加療中にBKPyVによる初感染を受けた可能性が示唆された。また出血性膀胱炎を発症していない状況で抗BKPyV-IgMが陽性化している症例もあり, これらの症例では尿中にBKPyVがPCRにより検出された。尿より検出されたBKPyVは塩基配列の相同性から複数の患者で同一株と判定された。相同性に加えて, 液性免疫応答からも初感染が複数の症例が院内で発生していると考えられる結果が得られた。さらなる症例の蓄積を継続し, BKPyV陽性化した患者の周囲環境からのBKPyV検出および分離ウイルスの不活化に関する検討を今後, 予定している。
なお, 今回, 一症例では造血幹細胞移植前より高コピー数のBKPyVが尿中より検出されており, 長期にわたり無症候性に排出がみられた。こういった現象の報告はこれまでになく, 水平感染の源になる可能性があり, より長期の経過をおって, 液性免疫応答との関連性も明らかにしていく。
This prospective clinical study was approved by the ethics committee and 39 patients who underwent allogeneic hematopoietic stem cell transplantation had already been enrolled into the study. Of the patients, 35 patients who survived longer than 3 months after transplantation were evaluable for the clinical course and dynamics of antibody titers against BK polyomavirus (BKPyV). In this study, virus-like particle without viral genome was used to prepare ELISA plate, and specific antibodies (IgG and IgM) against BKPyV were measured serially after transplantation. In addition, BKPyV genome was monitored and detected by using qualitative and quantitative PCR assays.
Of the evaluable 35 patients (median age, 48 years), 7 patients developed hemorrhagic cystitis due to BKPyV, whose median onset day was 23 days post-transplant. In 7 patients, 3 patient yielded positive results of anti-BKPyV-IgM antibody, suggesting the primary infection in these patients. In addition, a population of patients without clinical signs of hemorrhagic cystitis yielded positive results for anti-BKPyV-IgM antibody, and further evaluation detected BKPyV-DNA in the urine of these patients. The homology evaluation confirmed that some BKPyV isolates detected from different patients were identical. In addition to the homology evaluation, these results strongly suggest that humoral reaction also reinforces the possibility of horizontal infection of BKPyV among hematopoietic stem cell transplant recipients. The study is continued to accumulate the cases, and experiments of detecting BKPyV in the hospital environments and inactivation of isolated BKPyV are being prepared.
There was one asymptomatic case of persistent BKPyV viruria before transplantation with high copy numbers of BKPyV in the urine. Such a phenomenon has not been reported, and the possible role of such case in the development of nosocomial infection needs to be investigated. In addition, long-term follow-up is required and the association of humoral immunity in such a case should also be evaluated.
