本研究は、ラオス農村部の母子保健と保健衛生の改善に資するデジタル教材およびオンラインシステムを開発・評価することを目的としている。
ラオス国ルアンパバーン県チョンペ郡ノンチョン村が研究フィールドである。2019年11月24日～10月28日に、県病院およびヘルスセンターの医療従事者、妊婦および出産を経験した女性、小学校の教育関係者等を対象にフィールド調査を実施し、母子保健と衛生教育、インターネット環境に関するそれぞれの課題を整理し、2020年3月の実施と評価のため以下を準備した。
１．母子保健改善に資する教材作成・オンライン健診の実施可能性の検証
ラオ語の読解が不十分な妊産婦でも、母子手帳の必要な事項を理解し、妊産婦健診やワクチンを適時に受けることがきできるよう、音声・映像・画像を用いた「母子手帳活用サポートデジタル教材」を検討した。山間部では妊婦健診の受診率が低く、妊産婦死亡率が高いことが課題であった。そこで、農村部の妊婦が小型の胎児心音監視装置を装着し、オンラインで都市部の医師が胎児の心拍数と胎動、妊婦の子宮の収縮具合を診断することができる遠隔モニタリングシステムの導入の実証実験を計画した。
２．衛生教育の効果を高める教材
手指衛生・口腔衛生、蚊媒介感染感染の予防に焦点をあて、小学生やラオ語読解が不十分な住民でも、衛生知識と技術を適切に習得することができる教材を作成し、衛生行為の習慣化を促進する「衛生教育デジタル教材」を検討した。ノンチョン村小学校の生徒を対象にこれらの教材を使用して衛生教育を行ない、教育効果を評価することを計画した。
以上について、2020年3月8日～18日に現地で実施・評価を行なう予定であったが、新型コロナウィルスの感染拡大の影響により、直前に渡航が中止となったため、予定した期間内に実施ができなくなった。準備した計画は感染が収束した後、2021年3月に実施予定である。
This study aims to develop and evaluate digital educational materials and online system benefiting the maternal and child health and hygiene in rural areas of Laos.
The field of the study is Nong Chong Village, Chomphet District, Luang Prabang Province, Laos. From November 24, 2019 to October 28, 2019, the study conducted a field survey on healthcare workers at the provincial hospital and health centers, pregnant women and women with childbirth experiences, educators at elementary schools, etc. It then identified issues respectively related to maternal and child health, health education, and internet environment, and prepared as below for implementation and evaluation in March 2020.
1. Feasibility examination of educational materials and online health checkup benefiting maternal and child health
In order to enable pregnant and parturient women with limited Lao literacy to understand important matters in the maternal handbook and receive health checkups and vaccinations in a timely manner, the study examined the feasibility of "digital supporting material to utilize maternal handbook" featuring audios, videos and images. Poor prenatal checkup rate and high Maternal Mortality Ratio have been issues in the rural areas. Therefore, the study planned a demonstration experiment of a remote monitoring system, in which pregnant women in the rural area wear a small fetal heartbeat monitor and doctors in urban areas check online the fetal heartbeat and movements, as well as uterine contraction.
2. Educational materials to improve the effect of hygiene education
With a focus on finger hygiene, oral hygiene, and prevention of mosquito-transmitted infections, the study prepared educational materials to enable elementary school students and villagers with limited Lao literacy to acquire appropriate hygiene knowledge and skills, and examined the possibility of "digital hygiene education materials" promoting the establishment of hygiene practices. The study planned to conduct hygiene education on elementary school students in Nong Chong Village and evaluate the effectiveness of the education.
The study initially scheduled the implementation and evaluation in the field from March 8, 2020 to March 18, 2020, but the visit to Laos was cancelled just before the flight due to the global expansion of COVID-19. The implementation of the postponed plan is slated for March 2021, by which time the spread of the infection will have hopefully been curbed.
