メタボリックシンドローム(MetS)は内臓脂肪蓄積に高血圧・高血糖・脂質代謝異常が組み合わさり、動脈硬化性疾患をまねきやすい病態であり、食事、運動、睡眠、ストレスなどの生活習慣が大きく病態に影響する。一方、うつ病もメタボリックシンドロームと同様に生活習慣の影響を大きく受ける。メタボリックシンドロームとうつ病は相互作用することが明らかにされているが、メカニズムは依然不明なままであり、加えて、日本人を解析対象とする報告は限られている。本研究では、教職員健康診断データを用いてメタボリックシンドロームとメンタルヘルスの相互作用と、生活習慣との関連について解析を行った。保健管理センター独自に作成した、健康診断質問票により生活習慣(喫煙、飲酒、運動、食事、睡眠、ストレス、体重歴)について、メンタルヘルスチェックによりうつ病態について情報を収集した。2018年度教職員健診においてデータ提供の許可を得た2159人(信濃町地区を除く:匿名化したデータを用いた観察研究について登録番号201801 http://www.hcc.keio.ac.jp/ja/research/research-observational.html) 
の解析を行った。メンタルヘルスチェックはGoldberg による GHQ 精神健康調査票(The General Health Questionnaire-28;GHQ28)のうつ7項目から6項目を抜きだしたものであり、MetS非該当群、MetS予備群およびMetS該当群の3群の点数を比較した。メンタルヘルスチェック総スコアの中央値(範囲)は7(6-23)であった。ノンパラメトリックKruskal-Wallisによる検定では、MetS非該当群とMetS予備群には有意な差は認められなかったが、MetS非該当群とMetS予備群に比べ、MetS該当群は有意に総スコアが低くなっていた。また、総スコアと、喫煙、運動不足、間食習慣、睡眠不足、および高ストレスなど、MetSに繋がる生活習慣に正の相関が認められた。今回の評価ではMetSとうつ傾向に明らかな相関は認められなかったが、MetSとうつ傾向に共通する生活習慣が認められた。本結果を踏まえ、MetSとうつ病がお互いの新規発症に与える影響について今後検討する。
Metabolic syndrome (MetS) is a condition that combines visceral fat accumulation with high blood pressure, high blood sugar, and abnormal lipid metabolism, and is likely to lead to atherosclerotic diseases. Lifestyle habits such as diet, exercise, sleep, and stress greatly affect the condition. On the other hand, depression, like metabolic syndrome, is also greatly affected by lifestyle. Although it has been shown that metabolic syndrome and depression interact with each other, the mechanism remains unclear. In addition, there are only a limited number of reports analyzing Japanese subjects. In this study, I analyzed the interaction between metabolic syndrome and mental health and its relationship with lifestyle using data from health checkup of faculty and staff. We collected information on lifestyle (smoking, drinking, exercise, diet, sleep, stress, and weight history) using a health checkup questionnaire, and on depression using a mental health checkup. We analyzed 2159 individuals who gave permission to provide data(excluding Shinanomachi district: Observational Studies Using Anonymous Data, Registration Number201801, http://www.hcc.keio.ac.jp/ja/research/research-observational.html).
The mental health checkup consisted of six items extracted from the seven depression items of the GHQ28 (The General Health Questionnaire-28) by Goldberg. The scores of the three groups were compared. The median (range) of the total mental health checkup score was 7 (6-23). A nonparametric Kruskal-Wallis test showed that there was no significant difference between the non-MetS and pre-MetS groups, but the MetS group had a significantly lower total score than the non-MetS and pre-MetS groups. There was also a positive correlation between the total score and lifestyle habits that lead to MetS, such as smoking, lack of exercise, snacking habits, lack of sleep, and high stress. In the present study, there was no clear correlation between MetS and depressive tendency, but lifestyle habits common to MetS and depressive tendency were observed. Based on these results, the influence of MetS and depression on each other's new onset will be examined in the future.
