近年は核家族化や晩婚、晩産化など様々な事情を抱え、親を頼れない妊産婦が少なからずいる。妊娠・出産・育児を家庭のみに任せるのではなく、生活している地域で様々な関係機関や人が支援し、孤立を防ぐことが重要である。本研究の目的は、妊娠期からの切れ目ない支援の構築を目指し平成30年度に開始されたK市の産後ケア事業を評価し、その結果を元に今後のよりよい支援方策を検討することである。
評価の枠組みはDonabedianの医療の質評価モデル(ストラクチャー・プロセス・アウトプット・アウトカム)とした。まず、ストラクチャーとアウトプットについて整理するため、産後ケア事業に関わる職員体制、予算、委託先、利用者数、利用回数等を記載するシートを作成した。事業計画立案時に予算計上の根拠として算出した利用者数や利用回数と比較し評価する。次に、プロセスにあたる産後ケアサービスを利用したきっかけやサービスに対する満足度、利用後のフォロー電話の内容やタイミングについて利用者にたずねる自記式質問紙を作成した。結果をもとに、周知が十分であったか、サービスに関する満足度はどうであったか、満足度を上げていくためにはどのようなことができるか検討する。最後に、アウトカムに関係する産後ケア利用後の健康状態の変化や社会資源の利用状況、孤立についての状況等をたずねるインタビューガイドを作成した。インタビューの結果から産後ケア利用の効果についての内容を吟味し、定量的に効果を測定することの可能性を探索する。
研究の実施については、本研究の背景、目的、実施方法についてK市の母子保健主管課に事前に説明し、内諾を得た上で、研究者が所属する大学の倫理審査委員会による承認を受けた。実施段階となりK市母子保健主管課と当該課が属する健康福祉部間で話し合いが行われ、その結果個人情報の取扱方法と、研究協力者への謝礼に関する要望が出された。そのため、研究方法を一部変更することとなり、倫理審査委員会への変更申請が必要となったため調査の実施には至らなかった。次年度以降実施していく。
Recently, there have been a number of pregnant women who can't rely on their parents, having various circumstances such as becoming nuclear families, late marriages, and late births. It is important to support them by various related organizations and people in the area where you live for not let them alone. The purpose of this research is to evaluate the postpartum care project of K city which started in 2018 and to consider how to improve it based on the results.
The framework of the evaluation was the Donabedian model (Structure ,Process, Output, Outcome).
First, in order to organize the information of structure and output, I created a sheet for describing the number of staffs, the budget, the consignee, the number of users and the number of times of use etc. I will evaluate those data compare with the number of users and the number of times of use calculated as a basis for budgeting at the time of project planning. Then, I created a self-administered questionnaire which is asking the user about the motivation for using the postpartum care service, the degree of satisfaction with the service and the content and timing of the follow phone call after service use. Based on the results, I will consider whether the publicity was sufficient, how satisfied the service was, and what could be done to increase the satisfaction. Finally, I created an interview guide that asked about changes in health status after use of postpartum care related to outcomes, the state of use of social resources, the situation about isolation, etc. I will examine the effect of using postpartum care from the results of the interview and explore the possibility of measuring the effect quantitatively.
Regarding the conduct of the research, I explained about the background, purpose, and method of this research in advance to the head of Maternal and Child Health care Division of K city. After I obtained the consent, I got ethical approval from the ethics review board of the university to which I belong. At the implementation stage, discussions were held between the K City Maternal and Child Health care Division and the Health and Welfare Department and as a result, I got requests for handling personal information and rewards for interviewee from them. Therefore, I had to change research method and I could not conduct the research. I will carry out from this year.
