健康な青壮年が夜間就寝中に突然死する原因不明の病態であるポックリ病症候群は, 日本や東南アジアに多く, タイでは, Lai-Taiと呼ばれてきた。我々の現地での実態調査によって, タイ全土では, 年間1200～1800人程度がLai-Taiで死亡していると推計された。Lai-Tai事例について, 不整脈突然死をきたすBrugada症候群等に関係するSC5NA遺伝子解析をしたところ, 72%の事例で, 何らかのアミノ酸置換を伴う変異がみられ, その中には, Lai-Taiにのみみられるものもあった。これらについては, 今後機能解析を行う予定である。また, ミャンマーの訪問調査では, 類似の病態があるとの情報を得た。
Pokkuri Death Syndrome (PDS) is a sudden unexpected nocturnal death of unknown cause in mostly young males that has been reported in Japan and other South-Asian countries. We have been investigating Lai-Tai, similar to PDS, in Thailand with our counterpart. And from our study, we estimated that there is approximately 1200 to 1800 Lai-Tai death cases in the whole country. Genetic analysis of SC5NA, responsible for Brugada syndrome and other sudden arrhythmic death was performed in Lai-Tai cases. There were more than one amino acid substitutions in 72% of the Lai-Tai cases, and some mutations seemed to be peculiar to Lai-Tai, which need to be analyzed functionally. We also found similar cases in Myanmar in our collaboration study.
