| Abstract |
CTを用いて下肢リンパ管の立体解剖を明らかにした。また、リンパ浮腫におけるリンパ管の変化をCTにて捉えた。まず、予備実験にて未固定遺体のリンパ管の造影に適した造影剤を開発した。未固定遺体のリンパ管を造影し、CTで撮影することによりリンパ管の立体解剖を客観的に画像で捉えることが出来た。下肢のリンパ管の走行は人によって5つのパターンがあることが判明した。下肢に生じた癌の治療の際に行うリンパ節廓清の範囲を決定する際に、このパターンは重要な知見となる。また、リンパ浮腫では集合リンパ管の途絶や末梢のリンパ管への逆流が見られた。これはリンパ浮腫の病態解明の一助となる。
We elucidated three dimensional anatomy of the lymphatic systemn in lower leg, and lymphatic changes in lymph edema. First, we developed new contrast medium for lymphatic system in fresh cadaver. We injected the contrast medium to lymphatic system in human cadaer and imaged the lymphatic system by computed tomography. We found that five patterns of lymphatic flow are exsted in lower legs. This kwouledge is very important when surgeons decide the lyuphnode dissection area. And we found that some lymph collecting vessels are interrupded and contrast medium flew back to the lymph capillary. This helps understanding to the mechanism of human lymph edema.
