Abstract |
非穿孔式内視鏡的胃壁内反切除術(NEWS)+センチネルリンパ節ナビゲーション手術(SNND)の妥当性を理論的に証明するために以下の研究を行った。①NEWS+SNNS計11例における治療成績を解析した。全例において手技を完遂, 平均観察期間2.0年において全例無再発生存中であった。②内視鏡治療直後の早期胃癌検体表面を用いて捺印細胞診を行った結果, 癌幹細胞を含む癌細胞が接触により容易に遊離することが明らかとなった。③生体ブタを用いて内視鏡治療前後の胃リンパ流変化を検討した結果, 部位によってはリンパ流が改変される可能性があることが示唆された。④胃癌モデルマウスでは医原性性腹膜播種を認めなかった。
To investigate the usefulness of nonexposed endoscopic wall-inversion surgery (NEWS) with sentinel node navigation surgery (SNNS), the following studies were conducted. 1. In 11 cases of NEWS+SNNS, the procedure was completed in all cases, and 2 years' survival was quite favorable without recurrence. 2. Stump cytology on the surface of resected early gastric cancers revealed that cancerous cells were easily detached from the surface. 3. An in-vivo porcine study showed that the lymphatic flows of the stomach could be changeable after endoscopic resection according to the location of the lesion. 4. An animal study using gastric cancer model mice failed to demonstrate iatrogenic cancer cell seeding by opening the stomach with spontanesously developed gastric cancer.
