本年度は概ね計画通りに研究を遂行した。
少子高齢化が進む日本において、近年の経済状況は明るいものではない。その中において、医薬品産業は数少ない成長産業である。しかし、内資系企業の新薬創出はメガファーマと比較すると少ないという現実がある。
本研究においては日本発の医薬品創出を促進する基盤策定のための戦略を提示するという目的のため、規制産業である製薬産業が直面する日本の医薬品市場の特殊性に関して多面的な検討を加えた。まず、その検討のため、2006年以降のすべての新規開発医薬品に関して申請時データ及び薬価、売上高の変動を含めた新たなデータベースの作成を行った。
これにより、医薬品を一つの商品としてみることが可能となり、他の産業領域とは明らかに異なる市場特性を持つことを明らかにした。
本年度では、製薬会社の収益に大きく関わる薬価と、売上の変動に関して注目した。医薬品の承認申請時に製薬会社が想定していた当該医薬品の市場規模と、実際の売上との乖離を検証した。その結果、当初想定していた市場規模より売上規模が大きく上方乖離している医薬品が多くあり、その合計金額が2015年の段階で1年で1兆2000億円を超えることを明らかとした。日本の医薬品市場全体におけるこのような状況を示した知見は初めてのものであり、予測値から大きく乖離した場合、現行のシステムでは再算定という大幅な薬価切り下げが適用されることになっているが、その妥当性を検討する際の新たな重要な知見である。この結果を踏まえ、企業の収益性に関する検討を今後加える必要がある。本研究の成果は論文作成中であり、日本薬学会第140年会で共同研究者の中村教授とシンポジウムを開催して発表した（当該学会は、新型肺炎感染拡大のため、開催が中止となり、誌上開催となった）。また、研究成果を学生と討議するするブートキャンプを2020年2月29日に開催予定であったが、これも中止となった。
Japan is the one of the most promising countries for developing useful and novel drugs. The R&D activities of Japan-domestic pharmaceutical industries are strongly influenced by officially fixed drug prices. Predictions of drug sales play an important role in setting drug prices in Japan. Actual sales deviations from predicted amounts can induce disproportionate drug prices and unexpected pharmaceutical expenditures. This study aimed to estimate the upward deviation of actual sales from predicted sales and explore the predictors of such deviation.
Estimates of upward deviation in 2015 were produced for new molecular entities approved in 2006–2015. To identify the predictors associated with upward deviation through binary logistic regression analyses, sales within three years of launch and in the predicted year of peak sales were used.
The estimated upward deviation was more than 1,200 billion yen in 2015 for the targeted drugs. Drugs priced by the cost calculation method or by obtaining additional indications were significantly more likely to show an upward deviation from predicted peak sales.
This is the first report to show that there is a substantial upward deviation between actual and predicted drug sales in Japan. So long as drug sales predictions are used in drug price calculations, a flexible repricing system is needed to buffer unexpected pharmaceutical expenditures.
