難治がんの代表である膵癌に対する治療が模索される中、腫瘍間質の修飾により免疫治療効果が顕著に増強されることが明らかとなりつつある。本研究は、膵癌に対する免疫間質療法の実用化・個別化への展開の基盤となる成果を創出する。具体的には、患者組織の精緻な定量解析を用いて、ヒト膵癌においてT細胞性免疫の抑制に中心的役割を果たす間質因子・細胞の同定(目的1)、免疫抑制性微小環境に基づく膵癌サブクラス分類の提唱(目的2)を達成する。
我々は、細胞傷害性T細胞(CD8+細胞)の腫瘍内密度の定量測定法を開発し、これが予後の良好な指標となることを示し、国際学会にて発表した(2018年9月)。測定法開発を進める中で、CD8+細胞の癌組織内分布は不均一であり、膵癌中央部ではCD8+細胞浸潤が顕著に制限されていることを発見した。定量測定により、95%以上の膵癌症例で、腫瘍中心部のCD8+細胞密度は辺縁部に比べて低いことが証明された。さらなる検討で、腫瘍辺縁部では受動免疫反応が成立しているのに対し、膵癌中心部ではT細胞性免疫反応を打ち消す強力な免疫抑制性微小環境が存在し、結果としてCD8+細胞の腫瘍内浸潤が不均一に抑制されていることが示唆された。さらに膵癌中心部のCD8+細胞密度が低いほど患者生命予後が不良である一方で、辺縁部におけるCD8+細胞密度、癌細胞―CD8+細胞間距離は予後と関連がないことも明らかとなり、これらの結果を国内外の学会で発表し、原著論文を投稿した(in revision)。
本研究の成果により、①ヒト膵癌に対するT細胞性免疫の活動性に加え、②膵癌微小環境の持つ免疫抑制活性をin situで数値化する手法を概ね確立した。現在、これらの指標を用い、患者組織における癌間質治療のターゲットとなり得る腫瘍微小環境因子の候補を絞り込み、200例以上の膵癌臨床検体を用いて多重免疫染色切片の定量解析を進めており、研究計画は概ね計画通りに遂行されている。
The cancer stroma-modifying treatment against pancreatic cancer has emerged as a promising strategy to enhance the response to immunotherapy. This study aimed to identify the stromal factors/cells that may play a central role in the suppression of adaptive T-cell mediated immunity in human pancreatic cancer (Aim 1) and to propose the system of subtyping the immunosuppressive pancreatic cancer microenvironment (Aim 2) for the development of individualized therapeutic strategies targeting cancer stroma against this highly heterogeneous disease.
Firstly, we established computational quantitative system to calculate tumor-infiltrating CD8+ cells, and showed that CD8+ cell density in pancreatic cancer tissues was useful prognostic marker for patient survival (presented at the AACR Special Conference 2018). Then, we found substantial heterogeneity in CD8+ cell density, with the mean density in the tumor center less than half that in the tumor margin, probably due to the latent immunosuppressive factors within the pancreatic cancer microenvironment. Further analyses found that adaptive immunity-related findings correlated significantly with CD8+ cell density in the tumor margin, but not with that in the tumor center. Multivariable survival analyses revealed that the association of higher CD8+ cell density with prolonged survival was significant for the whole tumor; however, the association was stronger for the tumor center and insignificant for the tumor margin. Tumor cell–CD8+ cell distance correlated strongly with CD8+ cell density in the tumor center, whereas the density of CD8+ cells proximate to cancer cells exhibited no prognostic association. These results will be presented at the AACR Annual Meeting 2019, and in the original manuscript (in revision).
Consequently, our study has likely established the quantitative evaluation methods for the anti-tumor immune activity and for the in situ immunosuppressive activity of pancreatic cancer microenvironment. Using these quantitative measurements, we have identified candidate factors/cells contributing to the immunosuppression of pancreatic cancer stroma, and work is ongoing to quantify the levels of these candidates using multiplex immunohistochemistry-based image analysis on 200+ patient pancreatic cancer specimens.
