本研究課題に関連した主な実績は、以下の3点である。
第一に、革新的医薬品へのアクセスに関して、日本における薬価制度の現状や課題について整理ならびに分析を行い、日本の薬価制度改革の今後の方向性ならびに製薬企業の取るべき針路について考察した。その中で、薬剤費は中長期的には依然として上昇傾向にあること、ならびにドラッグ・ラグの再燃の懸念があることを提示したうえで、薬剤費上昇抑制とイノベーションの推進の両立を可能にするための４つのポイントを指摘した。さらに、今後の流れを踏まえ、製薬企業の取るべき進路について考察し、縮小する分野については事業の撤退／縮小を含めた戦略の見直しが求められ、今後伸びうる分野に対しては逆に積極的な投資が必要となることを示唆した。それら両方を進めることで、製薬企業は、薬剤費上昇抑制策に耐性を持つことが可能になる。
第二に、革新的な医薬品アクセスの現状に関し、ドラッグ・ラグの現状についてデータ収集ならびに分析を行った。一般的には、ドラッグ・ラグが解消されたという認識が依然としてあるものの、ドラッグ・ラグのうち、開発ラグについては、2013年以降増加傾向にある。そこで、開発ラグについて、日本、アメリカのデータベースから開発ラグに関するデータ収集を行うとともに、その増加要因の検証のために、医薬品ごとの特性（疾病分類、加算状況など）のデータを収集し、有用性、安全性に優れた医薬品がドラッグ・ラグになっていないかどうかの検証を行った。
第三に、革新的な医薬品アクセスを阻害する要因として、薬価に関するリスクを取り上げ、企業のリスク回避に関するサーベイ調査ならびにシミュレーションから、薬価に関するリスクを低減することで、どの程度研究開発インセンティブが高まるかを推定した。
The main achievements related to this research subject are the following three points.
Firstly, with regard to access to innovative medicines, I have analyzed the current situation and issues of a drug price system in Japan, and considered the future directions of the Japanese drug price system reform and possible courses pharmaceutical companies in Japan should take.
Secondly, data collection and analysis were conducted on the current state of drug lag, especially development lag, regarding the current state of innovative drug access.
Thirdly, we analyzed how to achieve higher R&D incentives without raising drug prices/expenditures, focusing on reimbursement drug price risk pharmaceutical firms face. For this purpose, we conducted an original questionnaire survey to collect data on attitudes toward risk of research-oriented pharmaceutical firms in Japan. With the collected data, we did numerical simulations to quantify the effect of reducing reimbursement drug price risk on those firms' R&D incentives.
