昨年度から引き続き、造血幹細胞移植後のBK polyomavirus (BKPyV)に関する前向き臨床研究への症例登録と各種検討を行った。移植後早期死亡例は評価対象から除外し、移植後3か月以上生存していた47例を評価対象とした。造血幹細胞移植方法の内訳は骨髄移植20例、末梢血幹細胞移植12例、臍帯血移植15例であった。本研究では内側にBK polyomavirus (BKPyV)のウイルスゲノムを含まないウイルス粒子構造を有する粒子(ウイルス様粒子：virus-like particle)抗原を固定した独自のELISA系を用いて患者血漿中の抗BKPyV抗体（IgG,IgM）を検出した。また尿中のBKPyVをPCRにより検出した。
47例中10例で移植後中央値48日（範囲：13-99日）にBKPyVによる出血性膀胱炎を発症した。その内、移植前抗体(IgG)陰性でBKPyV感染の既往がない症例2例もBKPyV出血性膀胱炎を発症していた。IgM抗体については移植後3か月間に移植前値の2倍以上、あるいはOD値1.0以上に急上昇した症例が10例認められたが、内7例は臨床的にはBKPyV感染症を発症していなかったため、感染症発症との関連性は確認できなかった。現時点で解析が終了している患者より検出された尿中のBKPyVの相同性に関しては、複数の患者で相同性が確認されている。
移植前抗体陰性患者からのBKPyV出血性膀胱炎が20%にみられ、また抗BKPyV-IgM抗体の有意な上昇が約20%の症例がみられ、出血性膀胱炎未発症例でもその変化がみられた。このことは移植後数か月という短期間に院内で水平感染という感染様式でBKPyV感染が起きていること、感染は起きても感染症発症には必ずしも至らないことが示唆される。また全例の解析が終了していないが尿中に検出されたウイルスの相同性も一致する症例があることもこのことを裏付ける所見と考えられる。
今後、さらなる症例の蓄積とウイルス相同性の検証を継続していき、より確固たる結果を確認する予定である。
This prospective clinical study about the BK polyomavirus (BKPyV) after allogeneic hematopoietic stem cell transplantation (HSCT) has been continued since last year. After excluding cases of early death, 47 patients surviving longer than 3 months could be evaluated. Types of HSCT were bone marrow in 20, peripheral blood in 12, and cord blood in 15. Virus-like particle without viral genome was used for ELISA evaluating specific antibodies (IgG and IgM) against BKPyV. BKPyV genome was attempted to detect by PCR.
Of the 47, 10 patients developed hemorrhagic cystitis due to BKPyV. Of the patients, two were BKPyV seronegative before transplant. The median onset was day 48 post-transplant (range, 13-99). In regard to the dynamics of IgM antibody against BKPyV, significant increase in the titer was observed in 10 patients. However, 7 of the 10 patients did not develop clinical hemorrhagic cystitis, and thus IgM elevation was not associated with the onset of hemorrhagic cystitis. Not examined in all patients yet, virus isolate homology was confirmed in some of the patients.
Of the patients with hemorrhagic cystitis due to BKPyV, 20% were seronegative before transplantation. In addition, significant increase in IgM titer against BKPyV was observed in 20% of the patients, and such change was observed even patients without hemorrhagic cystitis. These findings suggest that nosocomial horizontal transmission of BKPyV would occur in the early post-transplant period and the transmission does not necessarily result in the development of hemorrhagic cystitis. The results showing the homology of the isolates in some patients also reinforced the findings. Further accumulation of the cases and evaluations of the isolate homology are continued to obtain the more definite results.
