| Abstract |
外来における在宅療養支援について、病院内の患者に対する在宅療養支援に関するニーズ調査と外来看護師によるニーズ把握の実態に関する調査、外来における電話対応の実態調査、外来看護師の在宅療養支援に関する意識調査などを実施するとともに、病院内でのシステム構築を実践してそのプロセスを記述した。以上の結果を整理し、多くの病院で活用可能となるよう書籍としてまとめて公表した。
We conducted a survey on the needs of outpatients regarding home care support, a survey on the actual status of needs grasping by outpatient nurses, a survey on the actual status of telephone support in outpatients, and a survey on the awareness of outpatient nurses regarding home care support. In addition, the support system for outpatients was constructed in the hospital and the process was described. The above results were organized and published as a book that can be used by many hospitals.
