Food is likely to influence the effects of drugs and the development of side effects by altering gastrointestinal absorption of drugs. This factor includes changes in the gastrointestinal environment caused by food intake.
The gastrointestinal absorption of drugs classified as Class II (low solubility, high membrane permeability) in the Biopharmaceutical Classification System (BCS) is often increased by food intake. On the other hand, there are various types of foods, including high-fat foods, and enteral nutrients, in addition to standard foods. The effect of food on gastrointestinal absorption may vary depending on the type of food. Indeed, the absorption of rilpivirine, an anti-HIV drug classified as BCS Class II, is increased by food intake. However, the extent of this increase has been reported to be similar for standard breakfast and high-fat food, but different for enteral nutrients. On the other hand, the absorption of elvitegravir, a BCS Class II drug, also increases with food intake, but the extent of increase is reported to be similar between a standard breakfast and enteral nutrients. Despite the variety of patient eating patterns in clinical practice, there are limited reports evaluating whether the effect of food on gastrointestinal absorption of drugs alters by type. Therefore, the purpose of this study was to quantitatively evaluate the effect of food type on gastrointestinal absorption of BCS Class II drugs in small animals.
First, in verifying whether the results of human clinical trials could be reproduced in small animals using the above two drugs, I worked to establish a plasma drug quantification method. As a result, I was able to establish the conditions under which elvitegravir could be quantitated up to 3.0 µg/mL. However, this lower limit of quantification is expected to be insufficient for in vivo pharmacokinetic studies that mimic the estimated concentration in the gastrointestinal tract at clinical doses, and further investigation will be conducted.
