| Abstract |
本研究の目的は、精神看護専門看護師が裁量範囲を広げて実施することが期待される介入を特定し、ケアプログラムを開発、実践し、効果を検証することである。医師と看護師を対象にデルファイ法により介入を特定し、統合失調症患者の幻聴に対する行動マネジメントとがん患者のうつ傾向を改善するケアプログラムを開発した。統合失調症患者87名を対象に行動マネジメントプログラムの無作為化比較試験を、血液疾患患者30名、がん患者70名を対象にうつ傾向改善プログラムの前後比較、無作為化比較試験を実施したところ、ケアプログラムを用いた患者の方が有意に幻聴やうつが改善し、介入効果が検証された。
This study was to identify the care that Psychiatric and Mental Health Clinical Nurse Specialist (CNS) could expand their roles and to test effectiveness of care by CNS. At first we identified the intervention that other professionals expected CNS to implement through Delphi technique. Then three types of research on those interventions were implemented. One was randomized controlled study of behavioral management program for eighty-seven schizophrenic patients with auditory hallucination. In intervention group, patients could control their auditory hallucination compared with that of control group. The second research was quasi-experimental study for thirty blood cancer patients. Mental states of them were improved significantly. The third one was randomized controlled study for seventy cancer patients. Patients of intervention group were improved their depression and anxiety significantly compared with that of control group.
