Abstract 
腰部脊柱管狭窄症の重症例に対して行われる従来の後方除圧術では、手術侵襲に起因した術後の腰椎部傍脊柱筋の萎縮が高頻度に認められ、術後成績不良因子の一つとして考えられてきた。そのため、われわれは正中で棘突起を縦割して傍脊柱筋を極力温存して神経組織の除圧を行う「腰椎棘突起縦割式椎弓切除術(縦割法)」を開発した。本研究ではラット縦割術動物モデルを作製し、術後の組織学的変化、そしてreal time polymerase chain reaction (real time PCR)を用いて、筋組織委縮の進行と回復過程を検討した。組織評価とreal time PCRの検討の結果、筋委縮は縦割群で有意に低下しており、縦割法は術後筋組織の筋委縮を軽減できる手術手技の一つであることが示唆された。
