潰瘍性大腸炎関連腫瘍（ulcerative colitis associated neoplasia；UCAN）は様々な形態を呈するためその診断は困難とされていた。今回、特に診断が困難とされる平坦型UCANの内視鏡的特徴を明らかにするために検討を行った。UCANと診断された患者のうち、拡大内視鏡観察が施行された平坦型UCANを有する症例を対象に内視鏡所見の特徴を抽出し、2019年度からその特徴を有する粘膜面に対して更に詳細な拡大内視鏡観察を行った。また生検や内視鏡治療後の検体、手術検体による病理組織学的な対比も行い最終的に拡大内視鏡による平坦型UCANの内視鏡的形態学的特徴を明らかにした。
The diagnosis of ulcerative colitis associated neoplasia (UCAN) has been considered difficult because of its various forms. In the present study, we sought to clarify the endoscopic characteristics of flat-type UCAN, which is particularly difficult to diagnose.
Among patients diagnosed with UCAN, we extracted the characteristics of endoscopic findings in patients with flat-type UCAN who underwent magnified endoscopic observation, and from FY 2019, we performed further detailed magnified endoscopic observation of mucosal surfaces with these characteristics. In addition, histopathological comparison of biopsy, endoscopically treated specimens, and surgical specimens was also performed, and finally, endoscopic morphological characteristics of flat-type UCAN by magnifying endoscopy were clarified.
