市町村では特定保健指導や地域包括支援センターの運営等、外部委託により住民へのサービス提供が行われている。市町村保健師はサービスの質の向上に向け、委託側・受託側のそれぞれの立場や、意見の相違などから起きる感情に左右されずに「何が良いのか／どこに課題があるのか」を共有し、それについて「いつまでに・どのように・誰が・何を行うと改善されるのか」を共通認識できることが重要である。そのツールとして1時間程度で実施可能なマトリクス的な枠組みが有効である。特に「誰が」の部分は「保健師・受託事業者・住民」など具体例を示し、それぞれの立場で何をしたらよいのかをフラットに語れる表現で示しておくことが重要である。
