| Abstract |
日本の公的健康保険制度では, 3,000を超える保険者が分立しており, 各保険者が直面しているリスク構造や被保険者の所得状況は, 保険者間でかなりの差異がある。他方, 全ての保険者は, 国の法律によって定められた基準に合致するように給付を行わなければならない。このため保険者の財政状況には差異があり, 財政的に脆弱な保険者に対しては保険者間の財政移転が行われている。本稿の目的は, とくに組合健保に焦点を当てて, 保険者間の保険料率の差を示すとともに, 財政移転の規模と影響の長期的推移を明らかにすることにある。
