青年期の血圧は、潜在的な臓器障害や将来の高血圧・心血管疾患の発症に関連する。しかし、本邦において青年期の経年的血圧変化や年齢別血圧分布は不明であり、青年の血圧値を適切に解釈することが困難である。慶應義塾では、健康診断にて中学入学時から大学卒業時まで継続的に血圧測定を行っており、医療機関で収集が困難な健常青年の大規模かつ縦断的な血圧・健康診断情報を収集することが可能である。そこで、2023年度において、本研究では、慶應義塾の一貫教育という特色を活かした健康情報を利用し、青年期(中学生・高校生)の血圧における経年変化と年齢別血圧分布を評価した。
2000年から2019年の間に一貫教育校で実施した健康診断の身体計測ならびに血圧測定の結果を収集し、20年間分のデータを、5年間毎に4周期に分け、同一対象者からのデータ重複を避けるため、各周期で年齢がマッチするように層化無作為抽出を行った。経年分析として線形回帰分析およびロジスティック回帰分析を実施し、年齢別分布の評価のために中央値,90パーセンタイル,95パーセンタイルを算出した。20年間に健康診断を受けた延べ124460名を研究対象とし、各周期に3000名(計12000名)を層化無作為抽出した。全体の年齢は平均14.9歳で、男性が76.1%であった。男女ともに、20年間で身長に経年変化は見られなかったが、体重およびBMIの減少を認めた。男性では、収縮期血圧が経年的にわずかに低下したが(標準偏回帰係数 -0.029 [95%CI -0.050~-0.008],P=0.006)、拡張期血圧にはそのような傾向は見られなかった。女性では、収縮期血圧に変化を認めなかったが、拡張期血圧はわずかに上昇した(標準偏回帰係数 0.047 [95%CI 0.011~0.084],P=0.012)。年齢別血圧分布に関しては、男性では収縮期血圧,拡張期血圧が年齢とともに上昇したが、女性では年齢に伴う血圧値の変化は見られなかった。
In adolescence, blood pressure is associated with potential organ damage and the onset of future hypertension and cardiovascular diseases. However, the longitudinal changes and age-specific distribution of blood pressure during adolescence in Japan are unclear, making it difficult to interpret blood pressure values in youth accurately. At Keio Gijuku, continuous blood pressure measurements have been conducted from junior high school entrance to university graduation as part of health examinations, enabling the collection of extensive and longitudinal blood pressure and health examination data from healthy young individuals that is otherwise challenging to obtain in medical facilities. Therefore, in this study, we aimed to evaluate the longitudinal changes and age-specific distribution of blood pressure during adolescence.
Health examination results, including physical measurements and blood pressure measurements, conducted between 2000 and 2019 were collected. The data spanning 20 years were divided into four year-cycles of five years each, and a stratified random sampling was performed within each cycle to avoid data duplication from the same individuals, ensuring age matching within each cycle. Linear regression analysis and logistic regression analysis were conducted for longitudinal analysis, and median, 90th percentile, and 95th percentile were calculated for age-specific distribution evaluation.
A total of 124,460 individuals who underwent health examinations over the 20-year period were included in the study, with 3,000 individuals (total of 12,000 individuals) randomly sampled within each cycle. The overall mean age was 14.9 years, with males comprising 76.1%. Over the 20-year period, no longitudinal changes in height were observed for both males and females, but a decrease in weight and BMI was noted. In males, there was a slight decrease in systolic blood pressure over the years (standardized regression coefficient -0.029 [95%CI -0.050 to -0.008], P=0.006), while no such trend was observed for diastolic blood pressure. In females, no change in systolic blood pressure was observed, but diastolic blood pressure showed a slight increase (standardized regression coefficient 0.047 [95%CI 0.011 to 0.084], P=0.012). Regarding age-specific blood pressure distribution, systolic and diastolic blood pressure increased with age in males, while no significant change in blood pressure values with age was observed in females.
