視神経脊髄炎関連疾患(NMOSD)の新規治療薬候補となるCDC/ADCC活性を除去した抗アクアポリン4細胞外領域抗体(抗AQP4抗体)の安全性について、①初代培養ラットアストロサイトを用いたin vitroの系及び、② モルモットミエリン塩基性タンパク質により誘導される実験的自己免疫性脳脊髄炎(EAE)モデルを用いたin vivoの系で検討を行なった。
①については、AQP4の内在化を強く誘導するE5415Aと誘導しないE5415Bを用い、抗AQP4抗体の結合に伴うアストロサイト内での炎症性サイトカイン・ケモカインの発現に対する影響について評価を試みた。しかしながら、内在化を誘導する活性の有無によらず、抗AQP4抗体の結合によりアストロサイトでの炎症性サイトカイン・ケモカインの発現に変化は認められなかった。
②についてはこれまでに作製済みのE5415A(CDC/ADCC +・AQP4内在化 +)、E5415B(CDC/ADCC +・AQP4内在化 -)、E5415A mutant(CDC/ADCC -・AQP4内在化 +)に加えてE5415B mutant(CDC/ADCC -・AQP4内在化 -)を作製した。興味深いことに、E5415A mutantを用いてモデルを作製すると、この抗体はCDC/ADCCを誘導する活性を有さないにもかかわらず、E5415Aと同等のアストロサイト傷害が認められた。このことは本モデルにおけるNMOSD様病態において、CDC/ADCCに依存しない細胞傷害機構が存在することを示唆する。一方、E5415Bを用いてモデルを作製してもアストロサイト障害が現れるが、E5415B mutant(CDC/ADCC -・内在化 -)を用いた場合は血管周囲にミクログリアの集積が認められるものの、顕著なアストロサイト傷害は認められなかった。このことから、本モデルにおけるNMOSD様病態にはCDC/ADCCに依存しない細胞傷害機構が存在し、そのメカニズムに抗体依存的AQP4内在化が関与することが示唆された。
We investigated whether an antibody against the extracellular domains of aquaporin-4 (anti-AQP4 antibody) lacking functionality for CDC and ADCC, which is a potential candidate for a new therapeutic drug for neuromyelitis optica spectrum disorders (NMOSD), is harmful or not by ①an in vitro system using primary cultured astrocytes and by ②an in vivo system using experimental autoimmune encephalomyelitis (EAE) model rats induced by guinea pig myelin basic protein.
Regarding ①, we tried to estimate effects of binding of anti-AQP4 antibody to AQP4 on expression of inflammatory cytokines and chemokines in astrocytes, using two monoclonal antibodies against the extracellular domains of AQP4, E5415A (an antibody strongly inducing endocytosis of AQP4) and E5415B (an antibody inducing little endocytosis of AQP4). However, we did not see any alterations of expression of cytokines or chemokines, regardless of an ability of inducing endocytosis of AQP4.
As for ②, we produced new recombinant E5415B having multiple substitution mutations that eliminate functionality for CDC and ADCC (CDC/ADCC -, endocytosis -), in addition to E5415A (CDC/ADCC +, endocytosis +), E5415B (CDC/ADCC +, endocytosis -), and E5415A mutant (CDC/ADCC -, endocytosis +). Interestingly, injection of E5415A mutant caused loss of astrocytic markers comparable to that induced by E5415A, suggesting that a cytotoxic mechanism independent of CDC or ADCC is involved in the NMOSD-like pathology in this model. Although injection of E5415B also caused astrocyte loss, the mutant of E5415B did not, suggesting that antibody-induced endocytosis of AQP4 is involved in the cytotoxic mechanism independent of CDC or ADCC in the model.
