[1]PSCモデルマウスであるDDCによる薬剤誘発性胆管炎マウス、胆汁うっ滞性胆管炎を呈するMDR2 KOマウスにおいて、青黛投与により胆管障害が軽減することを見出した。AhR KOマウスを用いた検討では青黛の効果はAhR依存的であることが示された。また、AhR ligandであるIndigoおよびI3Cの投与でも胆管障害が同様に軽減することを見出した。さらに、タモキシフェン投与により腸管および胆管上皮特異的にAhRを欠損するマウスであるvillin cre ERT2/AhR floxマウスでの検討では、AhR ligandの胆管障害抑制効果はキャンセルされなかった。
[2]PSCモデルマウス肝臓内免疫細胞の解析では、ヒトPSC患者に特徴的である肝臓内IL-1b産生性マクロファージ・Th17細胞・IFNg産生性gδ陽性T細胞の浸潤がAhR ligand投与により抑制されることを見出した。また、腸炎モデルマウスにおける青黛の効果はIL22産生性ILC3細胞および制御性T細胞(Treg)などが関与するが、PSCモデルマウスにおけるAhR ligandの胆管障害抑制効果はILC3の欠如するRORgT KOマウスやTregの欠如するRAG2 KOマウスにおいても同様に誘導され、腸炎モデルとは異なった病態制御機構の存在が示唆された。
[3]PSCモデルマウスにおいて、腸管除菌にてAhR ligandの胆管炎抑制効果はキャンセルされ、AhRリガンド投与により胆汁酸合成経路におけるAlternative pathwayの合成酵素の活性化および代謝産物の増加が認められ、AhRligandの胆管障害抑制効果に腸内細菌および胆汁酸の関与が示唆された。
これら予備検討の結果から、AhRリガンドが新たな機構によるPSC病勢抑制効果を有する可能性が見出され、さらなる検討を進めている。先行研究で同定されたPSC疾患特異的腸内細菌Kpの死菌による刺激ではPSC患者末梢血単球のlL-1b産生が誘導されるが、本研究でAhR ligandのKp死菌刺激による単球lL-1b産生能抑制効果を検証する。
[1] In PSC model mice, including drug-induced cholangitis mice treated with DDC and MDR2 KO mice displaying cholestatic cholangitis, administration of Qing Dai was found to ameliorate bile duct injuries. Studies using AhR KO mice demonstrated that the effects of Qing Dai are AhR-dependent. Similarly, administration of the AhR ligands Indigo and I3C also resulted in comparable alleviation of bile duct injuries. Furthermore, in villin cre ERT2/AhR flox mice, which lack AhR specifically in the intestinal and bile duct epithelia due to tamoxifen administration, the bile duct protective effects of AhR ligands were not abrogated.
[2] Analysis of intrahepatic immune cells in PSC model mice revealed that administration of AhR ligands suppressed the infiltration of liver IL-1β-producing macrophages, Th17 cells, and IFNγ-producing γδ positive T cells, which are characteristic of human PSC patients. While the effects of Qing Dai in colitis model mice involve IL-22 producing ILC3 cells and regulatory T cells (Tregs), the bile duct protective effects of AhR ligands in PSC model mice were induced even in the absence of ILC3 in RORγt KO mice and Tregs in RAG2 KO mice, suggesting a different pathogenic control mechanism from that in colitis models.
[3] In PSC model mice, the bile duct protective effects of AhR ligands were negated by intestinal sterilization, and administration of AhR ligands was observed to activate enzymes of the alternative bile acid synthesis pathway and increase their metabolic products, suggesting the involvement of intestinal bacteria and bile acids in the protective effects of AhR ligands against bile duct injuries. These preliminary investigations suggest that AhR ligands may possess a novel mechanism for suppressing PSC progression, prompting further research. While stimulation with killed Klebsiella pneumoniae (Kp), a gut bacterium specifically associated with PSC, induces IL-1β production in peripheral blood monocytes of PSC patients, this study aims to examine the inhibitory effects of AhR ligands on monocyte IL-1β production capability following stimulation with heat killed Kp.
