藤沢市A地区に所在する団地に居住する全世帯(1,311世帯)の「各世帯の中で最も地域活動へ参加している/関心がある者1名」を対象に、調査票を配布した。268部(20.4%)の返信があり、その全てを分析対象とした。
先行研究を参考に、社会的フレイルの指標として「世帯構成(独居)」、「外出頻度(減少)」、「友人宅訪問(無)」、「家族・友人への貢献(無)」、「1週間のうち誰とも話さない日(数日)」の5項目をたずね、この5項目のうち2項目以上に該当する者を「社会的フレイルあり」と判定した。
対象者の平均年齢は71.8歳、女性が155名(58.7%)で、A団地での平均居住年数は23.6年だった。独居の者は152名(56.7%)、外出頻度が減少した者は70名(26.9%)、日ごろ友人宅等を訪問をしていない者は158名(64.8%)、貢献感が無い者は68名(27.4%)、誰とも話さなかった日が週に複数日ある者は79名(22.5%)で、社会的フレイルに該当する者は150名(58.8%)、うち、独居で日ごろ友人宅等を訪問をしていない者(以下、交流機会少群)は85名(31.7%)だった。
社会的フレイルに該当する者のうち、交流機会少群について、そうでない者との属性比較を行った。年齢が高いほど交流機会少群となりやすく(p<0.001)、主観的健康状態と経済状況が悪く(p=0.004, p=0.024)、日ごろの健康管理や外出、人づきあいに関心が低く(p=0.011, 0.043, 0.002)、孤立感を感じており(p<0.001)、相談相手が誰もないと回答する者が多かった(p=0.004)。利用している電子機器(スマートホン、PC等)や利用目的(情報収集やSNS利用)に有意差は示されなかった。
交流機会少群の者は、丁寧なアウトリーチが重要であり、インターネットでの情報収集やSNSを利用する者が約半数いるため、対面やチラシ配布だけでなく、オンライン上での情報発信や交流機会の効果的な提供方法について、今後検討が必要と考えられた。
A questionnaire was distributed to "one person in each household who is most involved/interested in community activities" in all households (1,311 households) residing in an apartment complex located in District A, Fujisawa City. 268 copies (20.4%) were returned, all of which were included in the analysis.
Referring to previous studies, we asked about five indicators of social frailty: household composition (living alone), frequency of going out (decreasing), visiting friends (none), contribution to family/friends (none), and days (several days) in a week when the respondent did not talk to anyone.
The average age of the subjects was 71.8 years, 155 (58.7%) were female, and the average length of residence in Apartment A was 23.6 years. 152 (56.7%) were living alone, 70 (26.9%) were going out less frequently, 158 (64.8%) were not visiting friends, etc. on a daily basis, 68 (27.4%) had no sense of contribution, 79 (22.5%) had multiple days a week when they did not talk to anyone, and 150 (58.7%) fell into the social frail Of these, 85 (31.7%) were single and did not visit friends' homes on a daily basis (hereafter referred to as the "socially frail" group).
Among those with social frailty, we compared the attributes of those who had few opportunities for social interaction with those who did not have social frailty. Older age was associated with fewer opportunities for social interaction (p<0.001), poorer subjective health and financial status (p=0.004, p=0.024), less interest in daily health care, going out, and socializing (p=0.011, 0.043, 0.002), a sense of isolation (p<0.001), and more likely to have no person to consult (p=0.001). Many respondents reported having no one to talk to (p=0.004). No significant differences were found in the electronic devices used (smartphones, PCs, etc.) or the purpose of use (information gathering, SNS use).
Since careful outreach is important for those in the group with fewer opportunities to interact, and about half of the respondents use the Internet to gather information and use SNS, it was considered necessary to consider how to effectively provide online information and interaction opportunities in addition to face-to-face meetings and flyer distribution in the future.
