An electrochemiluminescence-based assay for detection of anti-drug antibodies (ADAs) has been developed to explore the factors contributing to their development and their impact on clinical effectiveness. ADA and other biomarkers were assessed in patients with rheumatoid arthritis, examining their correlation with clinical outcomes. For infliximab, ADA positivity could not predict treatment persistence; while patients with lower infliximab levels and higher interleukin 6 levels had lower treatment persistence rates (Masui S, Biol Pharm Bull, 2023). Regarding tocilizumab, the ADA assay demonstrated a lower rate of ADA positivity in patients with rheumatoid arthritis compared to the other two drugs. Despite a higher ADA positive rate for etanercept, it had a lesser impact on serum drug levels and clinical response. Furthermore, we conducted a comparative analysis of the effectiveness and safety of etanercept between the reference product and its biosimilar in real-world settings, revealing a potentially superior safety profile for the biosimilar product. In summary, the development and impact of ADAs can vary across different antibody drugs. An exploration of the binding sites of ADAs on infliximab was also conducted. ADA-positive serum samples were spiked with intact infliximab or its fragments. The inhibition of ADA detection suggested the specific components of infliximab with corresponding epitopes. Additionally, etanercept underwent denaturation, reduction, and deglycosylation, followed by an assessment of changes in binding affinity against ADA. These results suggest that ADAs may recognize not only the amino acid sequence but also the higher order structures of antibody drugs.
In future research, we aim to identify factors such as ADAs that contribute to the pharmacokinetic profile of therapeutic antibodies through population pharmacokinetic analysis and other methodologies. The further analyses of ADA epitopes will focus on higher-order structural characteristics and molecular modifications of antibody drugs.
