看護管理の能力を高めることを目的に2つのプログラムを実施し、評価を行った。
1.「看護管理能力を高めるケースメソッド教育プログラム」の実践・評価
1)実施
参加者は、慶應大学病院看護師長15名(前年度からの継続10名、新規5名)。実施期間:2022年6月~2022年3月(全6回)。
第1回の導入講義の後、第2~5回はケース・メソッド学習計画によるディスカッションを行った。「コロナ禍の患者面会」「看護補助者と看護師の協働」「10年後を見据えた看護部教育構想」「組織デザインと意思決定」の4ケースを作成し用いた。毎回、客観評価(発言回数と発言時間)、主観評価(管理能力13項目等に関するWEB調査)を実施した。第6回は統合ディスカッションを実施した。
2)評価
看護管理能力の自己評価は、回を重ねる毎に高まっていた。しかし、統計的有意差はなかった。参加者が強化されたと回答したのは「多様な立場で考える力」(100%)、「問題・要因構造をつかむ力」(93.3%)、「意思決定力」(86.7%)、「組織を俯瞰する力」(86.7%)だった。一方、「説得する力」(40.0%)、「表現する力」「根拠を持って説明する力」「発想力」はいずれも53.3%だった。2021年度に比較し全ての力が相対的に高まっていた。これは2年目の参加者が66%を占めていたため、学習の継続効果と思われる。
2.「行動経済学の理論を活用した問題解決教育プログラム」の実践と評価
1)実施
参加者は慶應大学病院主任18名。参加者は第1回(2022年7月)ナッジ理論の講義後、約半年間、自部署でナッジを活用した業務改善を行いった。第2回(11月)中間発表、第3回(2023年3月)成果発表をおこなった。
2)評価
ナッジを活用した21の実践(業務整備:12事例、環境整理:7事例、感染予防:2事例)が行われ、実施前と比較して目標としていた成果が確認できた部署は約8割あった。しかし、慣れによる効果の減少も生じており、この改善が課題となった。
Two programs were implemented and evaluated for the purpose of enhancing nursing management competencies.
1. Implementation and evaluation of the "Case Method Educational Program to Enhance Nursing Management Competencies."
1) Implementation
Participants were 15 head nurses of Keio University Hospital (10 continuing from the previous year, 5 new). Implementation period: June 2022 - March 2022 (6 sessions).
After an introductory lecture in the first session, discussions were held in the second through fifth sessions using a case method learning plan. Four cases were created and used: " Family Visits to patients under COVID-19," "Collaboration between Nursing Assistants and Nurses," "Nursing Education Concept for the Next 10 Years," and "Organizational Design and Decision Making. Objective evaluation (number of times and duration of comments) and subjective evaluation (web-based survey on 13 management competencies, etc.) were conducted in each session. An integrated discussion was held in the 6th session.
(2) Evaluation
Self-evaluation of nursing management ability increased with each session. However, there was no statistically significant difference. Participants indicated that their "ability to think from diverse perspectives" (100%), " ability to analyze problems and factor structures" (93.3%), "decision-making skills" (86.7%), and "ability to catch the larger picture" (86.7%) had been strengthened. On the other hand, "ability to persuade" (40.0%), "ability to express," "ability to explain with evidence," and "ability to think out of the box" were all at 53.3%. This may be due to the effect of continued learning, as 66% of the participants were in their second year of participation.
2) Implementation and Evaluation of the "Problem Solving Education Program Utilizing Behavioral Economics Theory"
(1) Implementation
Participants were 18 chief nurses of Keio University Hospital. After the first lecture on nudge theory (July 2022), the participants improved operations using nudges in their own departments for six months. An interim presentation was made at the second meeting (November), and the results were presented at the third meeting (March 2023).
(2) Evaluation
Twenty-one nudge-based practices were implemented (improvement of work procedures: 12 cases, improvement of the work environment: 7 cases, infection prevention: 2 cases), and about 80% of the departments were able to confirm the targeted outcomes compared to those before the implementation. However, some reduction in effectiveness occurred due to habituation, and this improvement was an issue.
