わが国のう蝕や歯周疾患の患者数は589万人（2017年）で高血圧症に次いで多い。中でも歯周病は糖尿病や脳梗塞などの全身疾患の増悪因子となることも報告されており、薬局薬剤師が地域住民の口腔の健康維持・増進に関わる意義は大きい。諸外国では国民の口腔健康管理は薬局薬剤師の役割であることが浸透しているが、わが国の薬局薬剤師の認識や対応の実態は不明である。本研究では、薬局薬剤師を対象に来局者の口腔内症状に対する指導や相談応需の現状を調査し、薬局での口腔健康支援に対する認識や推進に向けた課題を明らかにすることを目的とした。2021年3月26日～29日に、薬局薬剤師を対象にweb方式自記式質問紙調査を行い（株式会社ネグジット総研に委託）、324件の回答を得た。薬剤による口腔関連副作用に関する指導や相談応需の経験は、口腔乾燥（78%）において最も多かった。一方、日常的な口腔トラブルについて、口内炎（70%）や口腔乾燥（55%）に関する相談応需経験のある薬剤師が多かったが、その頻度は「週に1回未満」が8割以上を占めていた。また、口内炎への対応に自信がある薬剤師は52%であった。薬局薬剤師の役割として賛同する割合は、口腔関連副作用への対応（88%）に比べて、日常的な口腔トラブルへの対応（66%）や口腔領域に関する薬局での取り組み（58%）では低下した。薬局薬剤師による口腔健康支援に対する不安として、「対応方法に関する知識不足」（60%）や「口腔内症状に関する知識不足」（55%）が多く挙がり、薬剤師向けの「口腔領域の教育・研修」（76%）のニーズが高かった。本研究より、わが国の薬局薬剤師の口腔領域に関する対応頻度は海外の報告と比べて少なく、自信度も低いことが示された。その一因として、口腔内症状やその対応方法に関する知識不足が挙げられ、薬剤師対象の口腔領域の教育・研修を構築することが必要であると示唆された。
In Japan, oral diseases, such as periodontal disease, are the second most common major disease behind hypertension. In addition, it is known that periodontal disease is related to systemic diseases, including diabetes mellitus and stroke. Therefore, it is meaningful for community pharmacists to maintain and promote the oral health of local residents. In foreign countries, it is recognized that community pharmacists play roles to manage the oral health of local residents. In this study, we examined about Japanese community pharmacists recognition of oral health support and handling against the oral health consultation. A web-based questionnaire survey was conducted with community pharmacists and 324 data were obtained. Among them, 78% have instructed patients about xerostomia, which is one of the oral side effects of medication. Although respondents received consumer enquiries about mouth ulcers (70%) and xerostomia (55%) as daily oral troubles, frequency of the enquiries was <1/week. Additionally, the ratio of those who had confidence to advice about mouth ulcers was 52%. Respondents considered that community pharmacies should play roles in handling oral side effects (88%) and daily oral troubles (66%), and in providing several services of oral health (58%). Respondents had anxiety about the lack of knowledge on handling methods against oral troubles (60%) and on oral disease symptoms (55%). In addition, the education and training about oral health managements were highly needed. These results showed that Japanese community pharmacists have fewer opportunities for receiving consultation about oral health from local residents and have lower confidence level about handling oral health problems than foreign community pharmacists. It was suggested that the education and training were needed for Japanese community pharmacists in order to extend the roles of maintaining and promoting the oral health of local residents.
