コンパッション(compassion:思いやり、慈悲)は、緩和ケアにおいて人間の尊厳を守り、質の高いケアを提供するために欠かすことのできない重要な要素である。コンパッションに基づく行動は、患者ケアだけでなく、医療従事者自身にとってもメンタルヘルスの改善やwell-beingの観点から、マインドフルネスと共に注目されている。しかしながら日本では、コンパッションの言葉自体に触れることも少なく、その要素やコンパッションを体現することについて検討されていない。
本研究は、緩和ケアに関わる医療従事者のコンパッションの理解について、マインドフルネスを含めた視点で調査することを目的とした。
対象は、レジリエンスと思いやりを構築する医療従事者へのマインドフルネスMindfulness for Health professionals building Resilience and Compassion: MaHALO :マハロブログラムに参加した医療従事者であった。研究デザインは、半構造化面接法によるフォーカスグループインタビューを用いて質的分析を行った。
インタビューには医師、看護師、PT、心理士などが参加し、コンパッションについては、「人間の根源的な共通性」「相互に関連する存在」「苦しみを癒す」「ケアする意図」「継続性と広がり」「自分と他者のwell-being」の6つのカテゴリが抽出された。またマインドフルネスとの関連からは、平静な態度・注意、俯瞰的な視点など認知・行動的な側面が見出された。
コンパッションは日本においても医療や看護、ケアにおける重要な要素として認識されていると共に、コンパッションをケアとして体現するためには、マインドフルネスによる実践が肝要であると考えられる。今回の知見をより深めるためには、引き続き継続した調査が必要である。
Compassion is an essential element in palliative care that protects human dignity and provides quality care. Behavior based on compassion has attracted attention not only for patient care but also for healthcare professionals from the perspective of improving mental health and well-being, together with mindfulness. However, in Japan, the word of compassion is rarely touched, and no consideration is given to embodying the element or compassion.
The purpose of this study was to investigate health professionals understanding of palliative care related to palliative care from a viewpoint including mindfulness.
Subjects were health professionals who participated in MaHALO: Mindfulness for Health professionals building Resilience and Compassion.
Doctors, nurses and psychologists participated in the focus group interview.
Six categories of compassion were extracted:the fundamental commonalities of human beings, the interrelated existence, the healing of suffering, the intention to care, the continuity and spread, and the well-being of oneself and others.In order to embody compassion as care, practice by mindfulness is essential.
