Abstract |
本研究では、日本の地方自治体における「健康な町づくり」の取り組みの歴史と、実践の中で育まれてきた「健康観」に関して医療人類学的視点から調査・考察を行った。具体的には、福岡・京都・大阪・長野といった先駆的試みを行ってきた地域医療の歴史について文献資料を調べ、関係者にインタビューを行い、健康な町づくりがどのように可能になったのかについての調査を行った。特に、「自己参加型」医療の台頭に着目し、戦前は必ずしも庇護的な意味をもたず、住民に警戒をもって見られることもあった医療的介入が、戦後各地でどのようにその意味を、より民主的で配慮的なものへと変えていったのかの分析を行い、その結果を4本の口頭報告・招聘講演と2本の執筆原稿として発表した。
調査は以下の三つの軸で進められた。1)福岡で戦後長年にわたり住民との協働の下に行われてきた久山町の疫学調査と、北九州での公害健診の歴史に着目し、住民運動・各自治体の自治との連動の中で、医療的監視がその役割や機能を変えていった経緯の一端を、文献調査とインタビューから明らかにした。2)京都や長野地域医療の先人たちの活動を文献で調べる過程で、従来の地域医療の歴史の中から、自然と認知症の地域医療が生まれてきた経緯が明らかになり、この動きが、現在の認知症に特化した地域医療とどのように連結するのかについての研究課題が浮かび上がってきた。この課題を受けて、3)地域における認知症医療を都市モデルとして進めている大阪の精神科病院等で調査を行い、認知症医療の新しい形が、住民との協力の下でどのように発展しているのかについての分析を行った。
今後は、大阪での調査から明らかになった、従来とは全く異なる神経科学的アプローチを用いた地域医療の在り方に着目することで、住民が当事者として臨床のみならず、科学知の生産にも寄与する新たな自己参加型医療の可能性について考えてみたい。
This study has examined the history of various local attempts to create "healthy communities" across Japan in order to explore the meaning of "health" from a medical anthropological perspective. By focusing on the four areas known for their active and rather distinctive attempts to create community medicine, I have investigated how doctor-pioneers in this field have tried to turn the meaning of medical intervention from something potentially oppressive to that which is protective and caring, and how local residents have become involved in the making of healthy communities.
The actual fieldwork and archival studies have been conducted with four areas and three themes in mind: 1) the rise of self-participatory medicine as a form of scientific collaboration in Fukuoka, which is worldly known for its long-lasting epidemiological study and the community medicine emerging at the height of industrial pollution; 2) grassroots community medicine in Kyoto and Nagano with pioneers who have helped define the meaning of "self-participatory" medicine in Japan; 3) the emergence of a new approach to community psychiatry in Osaka, which is currently experimenting with neuropsychiatric intervention for people living with dementia in the community.
Based on these investigations, I have given four oral presentations and written two papers; I will continue my future investigations with a focus on the emerging field of community neuropsychiatry in dementia medicine.
